Our Story
Diabetes Care Plus is a national service of Diabetes Victoria that brings together diabetes experts, professional training and development, as well as research support to strengthen diabetes care across Australia.
Back to the beginning
Our story began in 2023, when the Diabetes Victoria Clinic was established. This was in response to strong community demand for more tailored, diabetes specialist care.
From the beginning, the clinic provided people living with diabetes access to our specialist health professionals for tailored, person-centred support. As the service grew, it was realised that national organisations and individuals from across the country were seeking our diabetes expertise. This enabled us to expand beyond Victoria, offering our services nationally.
Alongside our community work, we saw an increase in demand for professional development. Health professionals, disability support workers, and aged care staff sought training to improve the way they supported people living with diabetes.
At the same time, our team was invited to collaborate in research trials, particularly those focused on lifestyle and diabetes management.
It was clear that the clinic had evolved into something more. It was most certainly a service with a broader scope and greater responsibility. We needed a name that reflected this growth: one that covered our national reach, professional services as well as our clinical services, and our commitment to diabetes care.
A new brand was born
Much consultation was sought and with our community’s help, Diabetes Care Plus was born.
The name reflects our values of doing more for people living with diabetes, for those at high risk of developing type 2 diabetes, and for the health and support workers who care for them.
The ‘Plus’ represents our values of empathy, innovation, and excellence, and our determination to offer more than just standard diabetes care. We’re committed to delivering compassionate support, evidence-based training, and meaningful contributions to research.
Our dedicated team of diabetes health professionals continues to provide stigma-free, person-centred care while staying on the front foot when it comes to the latest research, technology, and community needs.
At Diabetes Care Plus, we are proud to offer a person-centred, compassionate, and forward-thinking service, supporting individuals living with all types of diabetes and those at high risk of type 2 diabetes; training health professional, education staff and support workers; and contributing to research that advances diabetes care across Australia and worldwide.
