Booking an appointment at Diabetes Care Plus is simple!
You can be referred the following ways:
- Self-refer yourself; or
- Ask your GP, endocrinologist or any health professional for a referral to our service
- For any enquiries and to make an appointment, contact us: 1300 153 123 between 9am and 3:30pm Mon to Thurs
- Make a booking via our website below
Who is eligible to be seen in Clinic?
Please note we are diabetes specialised allied health clinic. We offer one-on-one appointments (including video calls) for adults (17+) with:
- type 1 diabetes
- type 2 diabetes
- MODY (Maturity Onset of Diabetes of the Young)
- LADA (Latent Autoimmune Diabetes in Adults)
- diabetes resulting from injury to or removal of the pancreas (type 3c)
- GDM (Gestational Diabetes Mellitus)
- adults at high risk of type 2 diabetes
If you don’t meet any of the above criteria and would still like to be seen in our Clinic, you may still be able to make an appointment in Cliniko. We assess referrals on a case by case basis, so please ask your GP to send a referral letter for you to info@diabetescareplus.org.au or speak to us on 1300 153 123
National Relay Service
Diabetes Care Plus welcomes calls through the National Relay Service (NRS). If you are d/Deaf, hard of hearing, or find it difficult to speak on the phone, simply choose your access option and provide our phone number 1300 153 123 when asked by the relay officer.
