At the Diabetes Clinic, we offer tailored personal support to help people living with: 

  • MODY (Maturity Onset of Diabetes of the Young)  
  • LADA (Latent Autoimmune Diabetes in Adults) 
  • diabetes resulting from injury to or removal of the pancreas (Type 3c diabetes) 

Our diabetes nurse educators and dietitians provide in-person and telehealth appointments. We support you with glucose monitoring, treatment understanding, carbohydrate counting, weight management, and reducing complication risks. We also offer insulin pump assessments and education.

How we can help you manage your diabetes

Our diabetes specialist health professionals can help with all aspects of management and care of your diabetes, which include:

  • Help you to understand how your diabetes developed in your body
  • How to monitor your glucose levels and what actions to do with out of target blood glucose levels
  • Help you to learn why the current treatment/s are recommended and support with taking them as recommended
  • Teach carbohydrate counting or check and support you with carbohydrate counting 
  • Helping you achieve your weight goals (including maintenance, gain or loss)
    • we offer weight neutral support
  • How to reduce your risk of diabetes complications 
  • Insulin pump clinic
    • For those on multiple daily injections – working out if an insulin pump is right for you
    • Starting an insulin pump 

National Relay Service

Diabetes Care Plus welcomes calls through the National Relay Service (NRS). If you are d/Deaf, hard of hearing, or find it difficult to speak on the phone, simply choose your access option and provide our phone number 1300 153 123 when asked by the relay officer.
