Collection and Privacy Notice
At Diabetes Care Plus, we value the privacy and security of our clients’ personal information. We are committed to ensuring that any personal information collected is handled in accordance with the relevant privacy laws, including the Victorian Privacy Principles.
As part of our ongoing relationship with you, we will collect certain personal information about you, including but not limited to:
- Full name
- Address
- Contact details (phone number, email address)
- Medicare number or DVA card number and/or health insurance numbers
- Medical and health information
The purpose of collecting this personal information is to support you to manage your diabetes/ high-risk of diabetes, process payments and provide client support.
We collect this information directly from you or from other sources with your consent, in compliance with the Victorian Privacy Principles. It is necessary for us to collect and retain this information to fulfill our obligations and provide you with the requested services.
Rest assured, we maintain appropriate safeguards to protect your personal information from unauthorised access, use, or disclosure. We do not share your personal information with any third parties except as required by legislation i.e. Medicare or with your explicit consent.

Glossary
- Cancellation Terms
Appointments that you do not attend, which are cancelled or rescheduled within 24 hours of the scheduled appointment time will incur a cancellation fee of $100. - Communication with referrer
Health professionals have an obligation to respond to the doctor or person who sent the referral. You consent to Diabetes Care Plus communicating with your referrer. - No emergency care: You understand that Diabetes Care Plus is not an emergency service, and you must book an appointment to access our services.
- Payment Terms: Payment is expected at the time of service. Clients will need to pay the full fee at the time of service; Medicare rebates are processed within 1-2 days. In the event of unpaid balances, we reserve the right to engage the services of a debt collector, with the associated fees to be borne by the client. You agree to these terms. For those on My Aged Care packages and NDIS service agreements we will invoice after service delivery.
- Respectful behaviour: Diabetes Care Plus provides a safe environment for both clients and staff. We reserve the right to end any consult early and discharge the client if there is aggressive or disrespectful behaviour or language.
