Please note – we cannot make dose changes during this appointment. We can identify whether adjustments are needed and either provide recommendations to your Endocrinologist or GP, refer you to our Diabetes Nurse Practitioneror schedule another appointment with one of our clinic DNEs and gain consent for DNE adjustments.

We’re here to identify if a ‘tune-up’ is needed. While we can’t change your doses during this specific chat, we’ll provide clear recommendations for your endocrinologist or GP, or refer you to our Nurse Practitioner to get the changes finalised.

These sessions are for assessment and strategy optimisation. While we identify where adjustments are needed, dose or setting changes must be finalised with your endocrinologist, GP, or Diabetes Nurse Practitioner.
The Services are being provided as part of a 6-month pilot program by MiniMed with Diabetes Victoria’s Diabetes Care Plus Clinic.
MiniMed are collecting and entering into a database, for the purpose of monitoring and completing due diligence on the Pilot Program, each Eligible Client’s name, postcode and date of birth.
The collected Eligible Patients’ data will not be used to market products, services, events and programs to the Eligible Patients. We will use the Eligible Client’s information in accordance with the MiniMed Privacy Policy.
The provision by Diabetes Victoria’s Diabetes Care Plus Clinic of the Services is not intended to influence the DNEs independent medical assessment and consultation of the Eligible client.
